Requesting Your Health Care Records
Clients have the right to request copies of their health care records. To request your records, please contact our office using one of the following methods:

We respond to record requests in accordance with Texas law and applicable privacy regulations, including the Texas Medical Records Privacy Act (Texas Health and Safety Code Chapter 181) and HIPAA.
Notice of Complaint Process: Texas Behavioral Health Executive Council
The Texas Behavioral Health Executive Council investigates and prosecutes professional misconduct committed by marriage and family therapists, professional counselors, psychologists, psychological associates, social workers, and licensed specialists in school psychology. Although not every complaint against or dispute with a licensee involves professional misconduct, the Executive Council will provide you with information about how to file a complaint.
